All staff members of Urology Clinic of Southwest Washington are required to sign a "Confidentiality Statement". Each of us is legally responsible for guarding privileged information and are subject to legal action if we divulge it. Breach of this agreement by any employee will result in immediate termination.
Therefore, messages will not be left with others (or on electronic devices) unless you have specifically requested we do so. Copies of chart documents will only be released upon signed authorization from the patient. If you have any questions, please let us know.
