| Office Hours and Appointments |
We are open from 9:00 am to 5:00 pm Monday through Friday. Patients are seen by appointment only. Urgent cases are seen as quickly as possible. Call at least 3 weeks prior to your desired time. New patients should arrive 15 minutes early. Bring a list of your current medications. |
| Other Appointment Issues |
We value your time and hope you value ours. Healthcare is unpredictable and can cause delays. You will be informed if the physician will be late. You will be given options for managing the delay. We request 24 hour advance cancellation notice. Managed care authorization should be confirmed prior to making your appointment. |
| Telephone Calls |
We encourage you to call during business hours with questions you have concerning your urological health care problems, prescription refill requests or test reports; however, it would be unfair to other patients if the doctor were to disrupt treatment to answer every phone call. Our staff is trained to answer most questions. If your call requires that the doctor speak with you, please leave a message and the doctor will return your call at the earliest opportunity. Emergency calls after hours will be taken by our answering service and forwarded to the physician on duty. |
| Prescriptions and Renewals |
Prescription refill requests must be made 2 days in advance during business hours. Requests will not be considered if the patient has not been seen within the past 12 months. Provide the name of the medication, dosage and the telephone number of your pharmacy. |
| Medical Records |
Please contact our office to transfer records from your current physician to us or to transfer our records elsewhere. You will be required to complete and sign a medical records request form. You can download a copy of this form by clicking on the tab "Download Forms". You can call us at (360) 823-1368 regarding medical records. |
| Transfer to Exam Table Policy |
Urology Clinic of SW Washington has a no-transfer / no-lift policy. We are not equipped with transfer aids such as electronic power lifts and do not want to jeopardize injuring our patients or our employees. We do have power tables that are low enough for self-transfers from wheelchairs. If full transfer support is required, we ask that arrangements be made to bring someone along to the appointment that can assist. In some cases, transfers require professional medical transports. The alternative is to be seen at a hospital where there is access to transfer equipment. |
| Electronic Medical Records |
In 2004, Urology Clinic of SW Washington, in its continuing commitment to quality patient care, implemented electronic medical records (EMR). Our EMR allows physicians quicker access to vital patient information and increases efficiencies in the clinic. Your provider has remote access to your records through a secure portal while away from the office. This enables us to communicate
quickly and accurately with other providers and local hospitals. |
