We share your concern about the rising costs of medical care. Our goal is to provide quality urologic health care services to our patients as efficiently as possible.
Charges incurred that are not covered by insurance carriers with whom we contract are due and payable at the time of service. Co-payments, deductibles and other personal balances must be paid at the time of service as well. We accept cash, check, debit cards, Visa, Mastercard, American Express and Discover. Charges for services not covered by insurance are to be paid two weeks prior to the scheduled procedure.
We file insurance claims only with those carriers with whom we contract. At the end of each visit, you will receive all the information necessary to file your insurance claim if you are covered by a private carrier.
If you have a question regarding insurance, our charges or billing policies, or your account, please ask a member of our business office staff and we will work with you to find the answer. Please understand that you are ultimately responsible for payment of fees charged for services rendered.
And remember, if you are covered by a managed care plan, please confirm your referral authorizations prior to each visit. If an authorization for your visit is not received prior to your visit, you will be asked to reschedule your appointment or sign a waiver indicating personal responsibility for the charges that day in the event the authorization is denied.
We are participating providers with Medicare and many major insurance carriers. For information about our provider network status with your carrier, contact the membership services number on your insurance card or check the carrier's website. |
